Paw Central Disclaimer
I, the undersigned (“I” or “Owner” or “You”), hereby acknowledge and agree that all the information provided in this application is complete and accurate to the best of my knowledge. I further acknowledge and agree I have read, understand, and agree to all terms and conditions by Paw Central LLC and its affiliates (“we,” “us,” or “Paw Central”) contained in the Paw Central general disclaimer (“Disclaimer”). Paw Central warrants that it and its agents will abide by all applicable laws and regulations and that the same will meet or exceed industry standards in their provision of services related to your pet-related to this agreement. I further attest that if I am not the sole owner or representative of the subject of this application, my signature is sufficient to enter into this agreement for and on behalf of any other owner or representative.
1. Release of Liability
I warrant that my pet is current with the vaccinations required by law, and that my pet is healthy to the best of my knowledge. If the pet is hurt or becomes ill, Paw Central has permission to call or take the pet to a veterinarian of our choice if pet’s vet is inaccessible: or administers medication: or gives other advisable attention, with our discretion and judgment, and such expenses will be paid promptly by the Owner unless injury is direct result of negligence of Paw Central. We will not be held responsible for any sickness or injury caused by the pet to itself. Paw Central is not responsible for any accidental death of pet of the nature of pre-existing medical conditions or natural disasters (fire, storm, flood, etc.). Paw Central will not be held responsible for clipper burn/minor nicks/de-matting/thinning/stripping/clipping resulting from grooming of matted, neglected coats or for irritation caused by removing coat from pets possessing mild to severe skin allergy, nor will we be held responsible for stressful effects grooming may have upon an elderly pet. If my pet bites or attempts to bite any person or non-human, a muzzle may be used, or at discretion of Paw Central staff. I will pay for all services rendered to that point and will pay the cost of all damages/expenses my pet has caused. Paw Central reserves the right to charge additional fees for services we consider over and above the norm covered by our standard rates. We also reserve the rights to refuse service to customers whose pet may pose a threat to us or to the other pets left in our care.
Whether it be an aggression problem, health problem, or parasite problems. If I fail to collect my pet at agreed time, I will pay the extra cost for boarding. The Owner agrees to be responsible for any property damages by pet.
2. Credit Cards Payments
Paw Central will only provide services to new customers once they have filled out a client card and disclaimer. This is essential in the unlikely event of any accidents or emergency while your pet is in our care. It is the client’s sole responsibility to inform Paw Central of any changes of contact information, billing card changes, or the pet’s health.
3. Failure to Sign
Failure to sign the disclaimer will result in services being refused. Paw Central will make no exceptions.
4. Cancellation and No Show Policy
We love caring for your pet. It’s fun! When someone doesn’t show up for their appointment, it’s not fun. It’s a problem. Our Policy is that clients who do not show up or cancel with less than 72-hour notice can’t make another appointment until they cover the full cost of the one they missed.
I agree that I have read and understand the Disclaimer for Paw Central. I understand that my pet, my guests, or I may be videotaped or photographed and that these images have become the property of Paw Central. I agree to release, hold harmless, acquit, will indemnify and forever discharge the corporation from all, and in all manner of action and actions, suits, damages, judgments, executions, claims, and demands whatsoever, in law or in equity, which I or any other person I have with me during services at Paw Central.
